We understand that saying goodbye to a beloved pet companion is one of the most difficult experiences a pet parent can face. When a busy clinic feels too impersonal, and an in-home visit simply isn’t possible, we’re here to offer another option for pet families.
Appointments must be booked at least 24 hours in advance.
To request an appointment, please complete our euthanasia request and release form. Dr. Smith will respond with available dates/times for an appointment within 12 hours of receiving your release form. Her fee for services is a flat rate of $450, this does not include cremations service fees.
